Application Forms

Application for the next intake (commencing study in Aug 2007) starts on 1 Oct 2006 and closes on 1 March 2007 for the Study Award Scheme route as well as for the Self-Financing route.

Those who are interested to apply for admission into the DMP, please download the Application Forms by the preferred route of entry.

For the preferred route of entry via Scholarship Scheme (SS), please click here for application forms. As for the preferred route of entry via the Self-financing (Personal/Direct Company Sponsorship), please click here for application forms.

After downloading, applicants should read through the important information contained within and ensure that the full set has been downloaded. Once verified, the document should be printed single side on A4 size white paper for proper completion.

We recommend that applicants download the application online. Those who prefer not to do so could do the following:

Online Request

Submit an online request for the application forms to be mailed to him/her using the online Enquiry Form. Full contact details including name, mailing address, email address and contact number should be furnished.

Email Request

Send an email request to the program administrator at tlidmp@nus.edu.sg for the application forms to be mailed to him/her. Full contact details including name, mailing address, email address and contact number should be furnished in the email.

Mail Request

Send a request for the application forms to be sent via postal mail. The request mail should be addressed to:

Full contact details including name, mailing address, email address and contact number must be furnished in the letter of request.

Note: Application forms requested for via the online form, email or postal mail will be sent via normal local mail (for Singapore addresses) or unregistered airmail (for foreign addresses). Applicants are advised to check the documents received to ensure that all are in order before starting to complete the form. Depending on the destination of the receiving party, please allow 3 days to 3 weeks for it to arrive. To avoid any delay, direct downloading of the application forms via the website, if possible, is highly recommended.
